As a Care Coordinator, you'll be the glue that holds our domiciliary care services together, ensuring everything is safe, effective, and up to scratch. Your responsibilities will include:

  • Supporting our Service Users 🀝
  • Managing our amazing Care Workers πŸ’ͺ
  • Keeping meticulous records ✍️
  • Making sure we're always compliant with CQC regulations βœ…
  • Ensuring top-notch continuity and quality of care for everyone we support ❀️

We're looking for someone with fantastic communication skills πŸ—£οΈ and serious organisational superpowers 🧠. You'll also need to be on point with GDPR, our company policies, and all contractual obligations with Local Authorities and private clients. πŸ’―

A critical part of this role is ensuring all new packages are brought on board safely and efficiently. This means you'll be responsible for confirming and evidencing:

  • We've gathered all the necessary information to understand individual needs. 🧐
  • Our team can safely meet each Service User’s needs. πŸ‘
  • We have enough staff with the right skills and continuity available. πŸ§‘β€βš•οΈ
  • A Field Care Supervisor can complete the Care Assessment within 72 hours. ⏱️
  • All contractual and operational requirements are met. βœ”οΈ
