  • Championing Support: Delivering high-quality care and outreach services to the people we support. 💖
  • Orchestrating Care: Managing individual care packages, keeping a close eye on delivered hours, and maintaining our banking hours system. 🗓️
  • Discovering Opportunities: Researching, planning, and scoping out exciting new activities and experiences for those we support. 🚀
  • Building Connections: Acting as a key worker, discussing support needs, identifying risks, and crafting comprehensive support plans and risk assessments. 📝
  • Community Link: Connecting individuals with fantastic opportunities within the local community. 🏘️
  • Sharp Record Keeping: Creating detailed care plans, risk assessments, and activity planners. 📊
  • Team Player: Collaborating with families, health professionals, and other agencies to provide the best possible support. 🌐
  • Leading the Way: Offering guidance and support to fellow support workers, including shift management where needed. 🧑‍💼
  • Clinical Expertise: Taking the lead on clinical aspects of service delivery as requested. 🩺
  • Flexible Support: Working within a floating support team to provide essential coverage. 🔄
  • Assessment & Referral: Contributing to the assessment and referral processes. ✅
  • Package Management: Overseeing a group of care packages, liaising with stakeholders, and managing staff. 💼
  • People First: Always prioritizing the rights and wishes of the people we support in all decisions. 💯
  • Financial Savvy: Adhering to all policies and procedures when making purchases, including the use of petty cash, and always aiming for value for money. 💰
  • Safety First: Immediately raising any concerns about the safety of those we support to a line manager. 🚨
  • Direct Support: Providing hours of care and support to individuals or groups as directed by your line manager. 🤲
  • Daily Task Allocation: Assigning daily work tasks to Support Workers as needed. ✍️
