• Care Planning: Collaborate with Service Users and staff to understand their needs and aspirations, helping to create personalized care plans that open up lifestyle choices. 📝
• Bringing Plans to Life: Put those plans into action, ensuring every Service User receives the support they need to thrive, always with their consent and focus on independence. 🌱
• Regular Check-ins: Work with the team and Service Users to review and update care plans, making adjustments as needed to ensure ongoing success. 🔄
• Admin & Teamwork: Keep accurate records, participate in team meetings to share insights and contribute to service improvements, and actively engage in your own development. 📊🤝

Key responsibilities

• Assists with and participates in discussions for care planning
• Implements agreed person-centred care plans for Service Users
• Participates in regular review and modification of care plans
• Maintains accurate and up-to-date records relating to services provided
• Attends and actively participates in staff meetings, briefings, and training activities
