This role is all about supporting adults with diverse needs, including mental health challenges, learning disabilities, Autistic Spectrum Conditions, and substance misuse. Think of yourself as a key player in their journey! 💯

What you'll be doing:

  • Providing amazing 1:1 and group support that actually matters. 🤝

  • Helping tenants crush their personal, social, leisure, learning, and employment dreams! 🎯
  • Championing independence in their daily lives. 🏠
  • Getting involved with shopping trips and mastering budgeting skills. 💰
  • Whipping up tasty and healthy meals with tenants. 🍳
  • Ensuring homes are safe, clean, and secure. ✅
  • Supporting tenants to attend important health appointments (GP, dentist, etc.). 👩‍⚕️
  • Inspiring tenants to dive into fun social and community activities. 🎉
  • Assisting with personal care when needed (with respect and dignity). 🤗
  • Boosting health and wellbeing every single day. ✨
  • Learning and assisting with medication procedures (after training, of course!). 💊
  • Contributing to individual Support Plans, Risk Assessments, and Health Action Plans. 📝
  • Helping tenants navigate tenancy responsibilities. 🔑
  • Carrying out essential health and safety checks. 🧐
  • Keeping super accurate records and support notes. ✍️
  • Filling out incident reports and other important docs. 📄
  • Chatting and collaborating with families, carers, care managers, and other professionals. 🗣️
  • Attending team meetings, supervision, and multi-agency events. 🗓️
  • Always following safeguarding, confidentiality, health and safety, and company policies. 🛡️
  • Kicking off mandatory and apprenticeship training to level up your skills. 📚
  • Constantly growing your knowledge, skills, and professional practice throughout your apprenticeship. 🌱

Key responsibilities

  • Provide 1:1 and group person-centred support to tenants.
  • Support tenants to achieve personal goals, independence, and positive outcomes in daily life.
  • Complete accurate records, support notes, and relevant documentation following support sessions.
  • Liaise with families, carers, care managers, and other professionals.
  • Adhere to safeguarding, confidentiality, health and safety, and other organisational policies and complete mandatory training.
